Before your integrative medical provider virtual consultation, the team at CKmd Lifestyle Nutrition will have you fill out your online nutritional health forms via our HIPAA-secure online platform or app. These forms must be completed prior to our team scheduling you with your CKmd Lifestyle Nutrition medical provider. It takes approximately 20-30 minutes to fill out these online forms.
